Payment

We require payment of your deductible, co-payment, co-insurance, and past-due account balances at the time of service. If you do not have insurance, full payment is required at the time of service.

We accept cash, checks, debit and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card with you to every visit. This will ensure that our records are kept current and accurate.

Our office participates with many insurance plans. We recommend that you call your insurance company, or go to their website to verify that we participate with your plan.

If you change your name, phone number, address or insurance plan, please notify us in writing with your signature for verification.
